Job Description

As a Triage Assistant at Lifelong Medical Care, you will play a vital role in our patient-centered team. Your primary responsibilities will include functioning as a liaison between the clinic Triage department and our Call Center, assisting walk-in patients with medication refills, and initiating paperwork processes that require clinical input from providers.

You will also schedule urgent and follow-up appointments in our electronic health record system, work closely with our Medication Program Coordinator to assist Contra Costa Cares and uninsured patients, and continually assess the triage system to identify problems and suggest improvements.

Requirements:
* High school diploma or equivalent
* One year of experience working in a healthcare setting or related coursework
* Proficiency in Microsoft Office, Internet, and electronic health records
* Excellent communication, problem-solving, and teamwork skills
