Job Summary

We are seeking a Licensed Practical Nurse (LPN) to join our interdisciplinary team at Providence PACE/ElderPlace. As an LPN, you will provide whole-person quality care to patients in need, working alongside a team to empower elders in your community to live active, independent lives.

Responsibilities
  • Provide direct patient care, including administering medications, performing vital signs, and providing wound care.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Conduct home visits to assess patient needs and provide education on self-care and disease management.
  • Participate in team meetings to discuss patient progress and develop strategies for improving care.
  • Document patient information accurately and thoroughly in the electronic medical record.
Requirements
  • Education sufficient for licensure as a Licensed Practical Nurse (LPN) in the state of Washington.
  • National Provider BLS - American Heart Association certification within 30 days of hire.
  • Washington Licensed Practical Nurse licensure upon hire.
  • 1 year of experience in long-term care or a related geriatric setting.
  • Experience working with the frail and elderly.
