LPN - PACE/ElderPlace Caregiver

As a Licensed Practical Nurse (LPN) on our PACE/ElderPlace care team, you will play a vital role in providing high-quality, patient-centered care to our elderly patients. You will work collaboratively with an interdisciplinary team to deliver whole-person care, empowering our patients to live active, independent lives.
Key Responsibilities:
• Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
• Collaborate with the care team to develop and implement individualized care plans that meet the unique needs of each patient.
• Conduct home visits to assess patient needs and provide education on self-care and disease management.
• Participate in interdisciplinary team meetings to discuss patient progress and develop strategies for improving care outcomes.
Requirements:
• Current Washington state LPN licensure.
• National Provider BLS certification within 30 days of hire.
• 1 year of experience in long-term care or geriatric nursing.
• Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
