Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our PACE/ElderPlace team in Seattle. As an LPN, you will work alongside an interdisciplinary team to provide high-quality care to patients in need. Our PACE/ElderPlace program is designed to keep older adults as healthy as possible through clinics, home visits, and other services.

Responsibilities
  • Provide whole-person quality care to patients in need, working alongside an interdisciplinary team
  • Empowered by our shared governance model, nurses are involved in creating practice standards and expectations
  • Work in a fast-paced environment, providing care to patients with complex needs
  • Collaborate with other healthcare professionals to develop and implement patient care plans
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care
Requirements
  • Education sufficient for licensure
  • National Provider BLS - American Heart Association within 30 days of hire
  • Washington Licensed Practical Nurse upon hire
  • 1 year Long term care or related geriatric experience
  • Experience working with the frail and elderly
