Geriatric Social Worker Job Description

We are seeking a compassionate and skilled Geriatric Social Worker to join our team at EAC Network. As a Geriatric Social Worker, you will provide essential care to the elderly population in the Town of North Hempstead.

Key Responsibilities:
  • Conduct comprehensive assessments to identify the needs of older adults and develop personalized care plans.
  • Assist clients with benefit and entitlement applications to ensure they receive the support they need.
  • Provide counseling and support to help clients cope with challenges related to aging.
  • Make referrals to community resources and services to help clients access the care they need.
  • Manage client records and statistics in line with HIPAA guidelines and Project Independence requirements.
Requirements:
  • Master's Degree in Social Work (MSW) from an accredited program.
  • Knowledge of Nassau County services and the specific needs of veterans and the geriatric population.
  • Strong verbal and written communication skills.
  • Ability to work with a laptop for documentation and proficiency in Microsoft Office.
  • Fluency in English required; Spanish, French/Creole, or Farsi is a plus.
  • Must have access to a car and a valid NYS Driver's License to travel within the Town of North Hempstead.
