Job Title: Medical Social Worker - Home Health

We are seeking a dedicated and compassionate Medical Social Worker to join our Home Health team at Jefferson Healthcare. As a Medical Social Worker, you will play a vital role in delivering short-term services to support patients and families in managing personal, financial, psychosocial, and environmental challenges related to their medical or psychiatric conditions.

Key Responsibilities:

  • Connect patients and families with valuable community resources to address their unique needs.
  • Provide emotional support and counseling to patients and families coping with illness or injury.
  • Assess patients' social and environmental needs and develop plans to address them.
  • Collaborate with healthcare teams to ensure comprehensive care and support for patients.

Requirements:

  • Washington State Licensed Clinical Independent Social Worker (LCISW) certification.
  • Two years of recent experience in a healthcare setting, preferably in a social work role.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a focus on delivering high-quality patient care.
