Job Title: Clinical Evaluator

Acentra Health is seeking a skilled Clinical Evaluator to join our team. As a Clinical Evaluator, you will conduct face-to-face assessments to facilitate appropriate placement and next level of care in Port Angeles, Washington.

Job Summary:

The Clinical Evaluator will provide essential evaluations to enhance patient well-being and improve their quality of life. These assessments, primarily conducted in nursing homes, hospitals, or community-based settings, play a crucial role in determining the level and scope of care required for each patient.

Key Responsibilities:
  • Conduct thorough medical record reviews and interact with healthcare professionals, patients, their families, and other support systems to ensure accurate screenings, evaluations, and determinations.
  • Assess and analyze psychological, emotional, behavioral, and physical functioning to develop informed level of care recommendations.
  • Collaborate with a diverse range of healthcare professionals, patients, families, and support networks to ensure effective care coordination.
  • Utilize independent judgment, deep clinical expertise, and exceptional communication skills to navigate complex situations and ensure the best outcomes for members, providers, and clients.
  • Stay updated on current regulations and research related to long-term care, mental illness, medical conditions, and geriatrics to ensure compliance with best practices.
Requirements:
  • Minimum 3 years of experience assessing individuals with mental or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive issues, and dementia.
  • Active unrestricted licensure in Washington in a healthcare or related field, such as a Licensed Psychologist, Psychiatric Nurse, Social Worker, Mental Health Counselor, Marriage and Family Therapist, or Social Worker Independent Clinical License.
  • Graduation from an accredited college with a relevant degree program.
  • Expertise in assessing and analyzing psychological, emotional, behavioral, and physical functioning.
  • Working knowledge of Preadmission Screening and Resident Review (PASRR) requirements per Federal Regulation 42 CFR, Chapter 483.
