Geriatric Social Worker Opportunity

EAC Network is seeking a dedicated and compassionate Geriatric Social Worker to join our team in the Town of North Hempstead. As a key member of our Geriatric Social Work Services program, you will play a vital role in providing essential care to older adults, empowering them to live independently and with dignity.

Key Responsibilities:
  • Conduct comprehensive assessments to identify the needs of older adults and develop personalized care plans.
  • Assist clients with benefit and entitlement applications, ensuring they receive the support they need.
  • Provide individual and group counseling to help clients cope with challenges related to aging.
  • Make referrals to community resources and services to support clients' needs.
  • Maintain accurate and confidential client records, statistics, and reports.
Requirements:
  • Masters Degree in Social Work (MSW) from an accredited program.
  • Knowledge of Nassau County services and the specific needs of veterans and the geriatric population.
  • Strong verbal and written communication skills, with the ability to understand and address the needs of older residents.
  • Proficient in Microsoft Office, email, and database software, with the ability to work with a laptop for documentation.
  • Fluency in English required; Spanish, French/Creole, or Farsi is a plus.
  • Must have access to a car and a valid NYS Drivers License to travel within the Town of North Hempstead.
