Role Summary:
Deliver Housing & Essential Needs (HEN) services to individuals identified as eligible by the Department of Social & Health Services, in alignment with the funding requirements from the City of Spokane. This role involves providing comprehensive case management to assist eligible individuals in formulating and implementing Housing Stability Plans while ensuring access to Essential Needs. Collaborate effectively with referral organizations and community partners to achieve optimal outcomes for HEN beneficiaries. A background check and driver record verification are mandatory.

Qualifications:
A Bachelor's degree from an accredited institution, or one to two years of relevant experience and/or training, or a suitable combination of education and experience.

Key Responsibilities:

  • Support participants in attaining housing stability.
  • Ensure participants receive necessary assistance to foster housing stability, which includes conducting needs assessments, collaborating with participants to develop tailored housing stability plans with specific objectives, monitoring progress from intake through program exit, and facilitating access to essential needs.
  • Document participant eligibility for the program. Assess household needs and priorities by gathering and recording pertinent information regarding income, public benefits eligibility, background, support systems, and living conditions to identify and coordinate effective services.
  • Coordinate and/or provide essential resources to help participants achieve and maintain stable housing.
  • Collaborate with community agencies delivering services under the HEN program to ensure successful implementation of Individual Housing Plans.
  • Utilize informal community networks to identify resources and make referrals as necessary to support Individual Housing Plans.
  • Engage in meetings, staff discussions, and other activities as directed to evaluate and enhance the effectiveness of Individual Housing Plans and overall case management efforts.
  • Contribute to the development and execution of programs and services.
  • Assist in screening referrals for suitable program and service options.
  • Establish and nurture positive working relationships with participants, referral agencies, and team members.
  • Monitor participant progress, providing motivation and support as needed.
  • Ensure all documentation and related activities are accurate, current, and compliant, including case notes and other required documentation.
  • Stay informed about state and federal regulations impacting the organization and the populations served.
  • Deliver services in accordance with organizational policies, CARF standards, and safety principles.
  • Assist in fostering natural support systems for participants.
  • Support participants in achieving the highest level of independence while receiving services.
  • Facilitate Person-Centered Planning to identify vocational goals, recommend appropriate services, and oversee agreed-upon programs and objectives, revising as necessary.
  • Demonstrate appropriate work behaviors and interpersonal skills.
  • Inform businesses, labor organizations, and the public about available services and potential employees.
  • Adhere to all organizational policies, procedures, and directives to ensure the safety of personnel and property.
  • Properly utilize and maintain all required Personal Protective Equipment (PPE).
  • Maintain confidentiality of agency information. Breaches of confidentiality may result in immediate termination.
  • Perform other duties as assigned.
