As a Case Manager, you will play a vital role in helping young people develop the skills and knowledge needed to thrive in adulthood.
Key Responsibilities- Develop and implement individualized plans to support foster youth in achieving their goals and transitioning into independent living.
- Provide guidance and support to youth in areas such as employment, housing, money management, permanency, and education.
- Recruit and retain program participants through outreach and engagement efforts.
- Conduct regular check-ins and progress monitoring to ensure youth are meeting their goals and making progress towards independence.
- Collaborate with other community service providers to ensure a comprehensive support system for foster youth.
- Document activities and progress in client records and complete accurate reporting according to agency and contract standards.
- Bachelor's degree in Social Science, Education, or related field.
- 1-3 years' experience working with youth, preferably in a case management or social work role.
- Excellent written and oral communication skills.
- Organizational ability with an attention to detail.
- Ability to multi-task and provide structured guidance and follow-through to help youth reach long-term goals.
- Conflict resolution and group facilitation skills.
- Interest in working both individually and as part of a larger team serving the same population.
- Basic computer skills in word processing, spreadsheets, and automated database.
- Ability to work independently with minimal direct supervision.
