Job Summary

Sea Mar Community Health Centers is seeking a skilled Case Manager to join our Care Management Department in Aberdeen, WA. As a Case Manager, you will play a vital role in providing support to patients and families, and technical or caseload support to the care management team.

Key Responsibilities
  • Meet with patients in their home, community, or clinic to provide support and technical assistance
  • Assist patients in achieving small steps toward their goals, including skill building and goal setting
  • Participate in groups such as Living Well with Chronic Conditions or Chronic Disease Self-Management Program
  • Advocate for patients with chronic conditions, understanding common chronic conditions and emotional issues
  • Handle patient information and activities in a confidential manner
  • Demonstrate professionalism and appropriate boundaries in all interactions
Requirements
  • High School Graduate or equivalent; Bachelor's degree preferred
  • One year paid or volunteer experience in medical, social service, or behavioral health case management or care coordination
  • WA State Community Health Worker Training and/or Chronic Disease Self-Management training
  • CPR certification or obtain it within the initial probationary period
  • Valid driver's license and proof of insurance
  • Ability to write routine reports and correspondence and speak effectively before groups
