PeaceHealth is seeking a Registered Nurse Minimum Data Set Coordinator for a Full Time, 1.00 FTE, Day position. The salary range for this position is $67.08. The hiring rate is influenced by various factors, including education, training, work experience, and applicable collective bargaining agreements.

This role is responsible for managing and supervising the comprehensive collaborative assessment and care planning process within the unit. The coordinator enhances the quality of care and services for residents through thorough assessment and analysis of quality indicators, as well as the development of performance improvement strategies. Ensures appropriate reimbursement through accurate and timely adherence to federal and state regulations.

Key Responsibilities
  • Organizes, develops, and coordinates the completion of documentation and patient assessment data as mandated by federal and/or state regulations for reimbursement. Ensures accurate data transmission in compliance with regulations and maintains a compliance log of all transmissions.
  • Leads the creation, implementation, and evaluation of individualized care plans for each resident. Facilitates interdisciplinary teams to identify resident needs and establish measurable care goals.
  • Certifies the accuracy of care plans and endorses them with their signature.
  • Updates and revises necessary documentation in response to changes in patient condition, adhering to federal and/or state guidelines. Effectively communicates with interdisciplinary team members regarding appropriate care levels and transitions.
  • Conducts audits related to reimbursement documentation and quality management processes. Reviews audit findings and provides summary information and recommendations for management review.
  • Develops and maintains an ongoing quality assurance program for resident assessments and care plans.
  • Ensures compliance from a risk management and regulatory perspective through early identification of clinical events or trends. Provides data management and summary information for benchmarking and comparative analysis.
  • Maintains current knowledge of federal and state regulations to ensure appropriate reimbursement. Educates and guides the interdisciplinary team on regulations, reimbursement systems, and associated documentation to maximize reimbursement levels.
  • Performs all duties in accordance with established policies, procedures, regulatory and accreditation requirements, and applicable professional standards.
  • Maintains and updates written policies and procedures governing the development and implementation of resident assessments (MDS) and care plans, in collaboration with the unit Director and/or Charge Nurse.
  • Performs additional duties as assigned.
Qualifications

Education
  • Bachelor's Degree Preferred.
Experience
  • Minimum of 3 years Required: Clinical experience.
Credentials
  • Required: Registered Nurse.
  • Required within 2 years: Resident Assessment Coordinator - Certified.
Skills
  • Strong communication skills are essential.
  • Excellent organizational skills and the ability to work independently are required.
  • Knowledge of quality improvement processes, particularly in clinical documentation standards, auditing, and reimbursement processes is preferred.
Working Conditions

Lifting:
  • Patient handling limited to 35 lbs. without assistive equipment.
  • Fine motor skills required for medical equipment handling and precise procedures.
  • Frequent pushing/pulling up to 45 lbs. force.
  • Occasional lifting (non-patient) up to 20 lbs. and frequent lifting of 5 lbs.
Environmental Conditions:
  • Exposure to biohazards, body fluids, and airborne particles.
  • Ability to perform tasks in a noisy environment.
Mental/Visual:
  • Vision and hearing within normal limits (corrective devices permitted).
  • Ability to communicate and exchange accurate information.
