About the Role

We are seeking a highly skilled and dedicated Licensed Vocational Nurse (LVN) to join our team as a Charge Nurse at Park Manor of West Chase. As a Charge Nurse, you will play a vital role in ensuring the delivery of high-quality patient care and leading our nursing team to achieve exceptional outcomes.

Key Responsibilities
  • Supervise and coordinate the effective use of personnel, supplies, equipment, and facilities to ensure the efficient delivery of patient care.
  • Review staffing patterns and census of nursing units, reassigning personnel as necessary to meet resident needs and budgetary controls.
  • Manage and analyze patient care following admission or readmission, conducting initial assessments for newly admitted or readmitted patients.
  • Assist in creating and implementing care plans that address individual residents' needs, ensuring that care is safe, efficient, and therapeutic.
  • Administer medications, IV therapy, gastronomy tube management, and wound care as needed.
  • Monitor residents and communicate changes to physicians, ensuring that care is tailored to individual needs.
  • Ensure accurate documentation of resident care and maintain medical records, adhering to regulatory requirements.
  • Troubleshoot and investigate complaints, taking necessary action to resolve problems and maintain a positive work environment.
Requirements
  • LVN/RN License in good standing.
  • 1 year of experience as a nurse, with experience in a long-term care facility preferred.
  • Effective communication skills, with the ability to organize, prioritize, and maintain confidentiality.
  • Ability to react decisively in emergencies and provide hands-on training to nursing staff.
  • Exceptional supervisory skills, with a proactive approach to developing and leading the nursing team.
  • Good problem-solving and independent thinking skills, with a focus on case management and client relations.
