Job Summary

We are seeking a highly skilled Licensed Vocational Nurse I to join our team at Healthcare For the Homeless. As a key member of our healthcare team, you will provide clinical support and patient care coordination to ensure the highest quality of care for our patients.

Key Responsibilities
  • Clinical Support
    • Provide telephone advice to patients with minor acute problems, following established protocols and guidelines.
    • Assist with monitoring acutely ill patients during patient visits and perform various procedures within the scope of LVN practice.
    • Participate in patient follow-up activities, including reporting diagnostic test results, arranging prescription refills, and expediting referrals.
  • Patient Care Coordination
    • Coordinate patient care activities, including tracking and supporting Patient Assistance Program (PAP) drugs, and assisting with medication management.
    • Participate in patient care quality improvement projects and contribute to the development of training materials for clinical support staff.
    • Assist with maintaining stocks of clinical supplies and medications, and perform chart audits to identify opportunities for training.
  • Team Collaboration
    • Work collaboratively with healthcare teams, including medical assistants, providers, and other support staff, to provide patient education and support.
    • Participate in on-going management of patients with stable chronic diseases, communicating with patients and coordinating treatment plans.
Requirements
  • Current license as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) by the State of Texas Board of Nurse Examiners.
  • Basic Life Support certification (BLS).
  • High School diploma or G.E.D and graduated from an approved licensed practical nurse/vocational nurse (LPN/LVN) program.
Preferred Qualifications
  • Experience with Patient Centered Medical Home principles and quality processes.
  • Experience and/or interest in working with homeless or other underserved populations.
