About the Role

This position involves assessing, developing, implementing, and evaluating nursing care for assigned patients. As a key member of our multidisciplinary team, you'll work closely with other disciplines and agencies to ensure comprehensive patient care.

Responsibilities

  • Promotes effective communication among healthcare professionals regarding patient care and agency functions, documenting issues and interventions accordingly.
  • Maintains accurate records of patient assessments, discharge planning, physician orders, and care plan coordination throughout certification.
  • Fosters strong relationships with medical staff, upholding professionalism and adhering to agency policies and procedures.
  • Conducts regular assessments and initiates interventions as needed, adjusting plans based on changes in patient condition.
  • Develops personalized discharge plans addressing ongoing patient needs before discharge, ensuring confidentiality and maintaining an acceptable caseload according to agency policy.
  • Ensures timely submission of documentation and participates in chart audits and quality improvement projects.
  • Takes on assignments beyond standard duties when required, supporting the agency's needs.

Requirements

  • An Associate's degree in Nursing is required, along with at least one year of experience as a Registered Nurse.
  • A valid Registered Nurse license in South Carolina is essential, as well as a valid driver's license, a satisfactory three-year motor vehicle record, and a certificate of auto insurance.

Work Environment

This role requires reliable transportation and adherence to Prisma Health's Driver Safety Policy and department driving policies. A clean driving record is necessary, and passing a driver training program is required.

Salary Information

The estimated salary range for this position is $65,000 - $85,000 per year, depending on experience and qualifications.
