Job Summary:

We are seeking a skilled Registered Nurse to join our team as a Nurse Care Coordinator for Cardiac Patients at Prisma Health Heart Hospital.

About the Role:

This is a dynamic and challenging position that requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. The successful candidate will be responsible for coordinating care for patients with cardiac conditions, including congestive heart failure, myocardial infarction, and cardiac arrhythmias.

Key Responsibilities:

  • Assess patient needs and develop individualized plans of care
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Provide education and support to patients and their families
  • Maintain accurate and timely documentation

Requirements:

  • Associate's Degree - Nursing
  • One year of experience in Nursing as a Registered Nurse
  • SC Licensed RN - Registered Nurse
