POSITION SUMMARY:

The Clinical Advancement Registered Nurse (CAP RN 1) serves as an entry-level role within the Clinical Advancement Program for Registered Nurses. This position is essential for delivering and coordinating high-quality clinical care, which includes assessment, planning, intervention, and evaluation for a designated group of patients.

As a vital member of the Kidney Disease healthcare team, the CAP RN 1 engages in decision-making processes, educational initiatives, leadership responsibilities, and quality improvement efforts aimed at enhancing patient care outcomes and operational efficiency.

KEY RESPONSIBILITIES:

  • Execute all fundamental functions under the supervision of the Program Manager and with support from the Educator, Preceptor, or an experienced CAP RN for the initial months of employment.
  • Aim to fulfill the requirements necessary for progression to the next Inpatient CAP level within a specified timeframe from the start date.
  • Conduct continuous and systematic collection and analysis of patient data before, during, and after treatment for assigned patients, ensuring thorough documentation in the medical record, and adjust treatment plans as necessary.
  • Identify variations in patient status and collaborate with relevant healthcare team members for input, including management and attending physicians.
  • Assess daily patient care needs and work collaboratively with both direct and ancillary care staff as required.
  • Provide safe and effective patient care as prescribed for all specific treatment modalities.
  • Facilitate communication with both FKC and non-FKC dialysis providers and hospital personnel to ensure continuity of care.
  • Administer medications as per physician orders and in compliance with contractual agreements.
  • Ensure that nephrology physician orders for assigned patients are accurately recorded in the Medical Record.
  • Document all treatment data meticulously and promptly in the designated medical record.
  • Implement, monitor, and document patients' responses to prescribed intradialytic transfusions, including notifying physicians of any adverse reactions.
  • Familiarize oneself with company policies and emergency protocols to effectively assist in emergency situations.
  • Delegate appropriate tasks to support staff, including Patient Care Technicians and LPNs.
  • Monitor patient care staff to ensure adherence to policies and procedures.
  • Promote infection control and maintain a safe environment.
  • Assess and document the educational needs of patients and families to provide ongoing education.
  • Instruct facility staff on specialized care related to renal therapies.
  • Collaborate with the interdisciplinary team to update nursing assessments and care plans.
  • Participate in quality assurance initiatives and complete required reports as directed.
  • Serve as a resource for the healthcare team and assist in training new staff as assigned.
  • Attend all mandatory staff meetings as scheduled.
  • May be required to assist in outpatient facilities as needed.
  • Uphold and promote the mission, vision, values, and customer service philosophy of the organization.
  • Perform additional duties as assigned by management within regulatory standards.

WORK ENVIRONMENT AND PHYSICAL DEMANDS:

  • The physical demands and environmental conditions typical of this role may include heavy lifting, moving patients, and assisting with ambulation. Reasonable accommodations may be made for individuals with disabilities.
  • Areas of practice may include hemodialysis, peritoneal dialysis, continuous renal replacement therapies, and apheresis.
  • The role involves direct patient care and may require frequent standing, bending, and lifting of equipment and solutions.
  • Daily responsibilities include desk work, computer tasks, and interaction with patients and healthcare staff.
  • Participation in an on-call rotation may be required.

SUPERVISORY RESPONSIBILITIES:

  • Oversight of Patient Care Technicians and LPNs may be assigned after meeting specific training and experience requirements.

EDUCATIONAL REQUIREMENTS:

  • Must be a graduate of an accredited School of Nursing.
  • Current state licensure is required.

EXPERIENCE AND SKILLS:

  • Entry-level position for RNs with less than two years of nephrology nursing experience or RNs with a BSN and less than one year of nephrology experience.
  • Preferred minimum of nine months experience as a Registered Nurse.
  • Six months of nephrology experience is required before independent practice.
  • Successful completion of all required training and certifications is mandatory.
