Job Summary

We are seeking a highly skilled and compassionate Bilingual Patient Navigator to join our team at One Community Health. As a key member of our healthcare team, you will play a vital role in linking immigrant, refugee, and non-English speaking patients with essential health, psychosocial, and other services.

Key Responsibilities
  • Provide comprehensive health navigation, including assistance with health insurance enrollment, registration, appointment scheduling, and interpretation of forms and paperwork.
  • Coordinate with external immigrant and refugee services to create partnerships and help identify and connect patients with our services.
  • Deliver patient education through printed materials, group health workshops, telephonic, and verbal interactions.
  • Assist with outreach telephone calls to reduce care gaps and keep patients engaged in care.
  • Remain current on health-related community resources and act as a point of contact for patients.
Requirements
  • High School Diploma or GED.
  • At least 2 years of customer service experience.
  • At least 1 year experience working with diverse populations, including refugees and immigrants.
  • Fluency in Dari, Pashto, and English, with the ability to read and write in these languages.
  • Understanding and sensitivity to the cultural nuances and practices of Afghan communities.
Preferred Background
  • Prior experience working in healthcare settings, preferably in a role involving patient interaction or navigation.
  • Existing knowledge of external, local resources and services for immigrants and refugees.
