Job Summary

The Patient Care Supervisor is a key member of our team at Excelin Health Services, responsible for providing administrative, planning, and coordination assistance to our Director of Patient Care Services. This role involves supervising Home Health Aides, Licensed Vocational Nurses, and administrative staff, as well as collaborating with agency departments, facilities, and physicians to coordinate appointments and obtain necessary documentation.

Key Responsibilities:

  1. Oversee the Scheduling Department, ensuring accurate employee and patient schedules, and staff productivity.
  2. Participate in the interview process for new hires and have the authority to make hiring and firing decisions.
  3. Supervise the scheduling department to verify accuracy and reconcile schedule variances.
  4. Approve and monitor missed visits, ensuring timely rescheduling and staffing.
  5. Run daily, weekly, and monthly reports to track performance and identify areas for improvement.
  6. Support patient needs by communicating with field staff and prioritizing assignments based on patient condition.
  7. Ensure compliance with State and Federal Regulations by reviewing orders and providing guidance to staff.
  8. Assist in maintaining patient satisfaction and improving ratings.
  9. Perform other duties as assigned, including covering field staff during high census situations.
  10. Ensure personnel development, including orientation for new staff.
  11. Facilitate patient care coordination documents with acute and post-acute facilities during transfers.
  12. Assist with care coordination, including DME, medical supplies, and appointment referrals.
  13. Answer incoming calls, triage medical issues, and document complaints.
  14. Investigate and resolve patient complaints, escalating to the DPCS/Branch Director if necessary.
  15. Instruct and guide clinical staff to promote effective performance and quality care.
  16. Lead case conference meetings to facilitate care coordination.
  17. Direct Emergency/Disaster Triage.
