Overview:

At AccentCare, we're seeking a skilled Sales Associate to join our team. As a key member of our sales team, you'll play a vital role in facilitating the home health referral process and coordinating care for patients in their homes.

Responsibilities:

  • Facilitate the home health referral process and coordinate information with patients, family members, physicians, and other involved entities.
  • Assist in providing home care staff with necessary information on home care plans, ordering equipment, and communicating with the home health agency.
  • Secure physician orders to initiate care and facilitate face-to-face documentation.
  • Provide the home health agency with complete referral information to ensure timely and appropriate care.
  • Participate in SNF/hospital coordination meetings and lead clinically based in-services for referral sources and patients.

Qualifications:

  • One year of home health experience preferred.
  • Must be a licensed driver with an automobile that is insured and in good working order.
