Position Summary:
The nurse care coordinator plans, organizes, and directs home care services, focusing on community health education and experience. This professional nurse builds on community resources to plan and direct services that meet the needs of individuals and families within their homes and communities.
Key responsibilities include:
- Completing initial assessments of patients and families to determine home care needs.
- Providing physical assessments and histories of current and previous illnesses.
- Regularly re-evaluating patient nursing needs and initiating necessary revisions.
- Developing care plans that establish goals based on nursing diagnoses and incorporate therapeutic, preventive, and rehabilitative nursing actions.
- Administering medications and treatments as prescribed by physicians.
- Counseling patients and families on meeting nursing and related needs.
- Providing health care instructions to patients as appropriate per assessment and plan of care.
- Identifying discharge planning needs as part of the care plan development and implementing prior to discharge.
- Acting as a case manager when assigned by the clinical manager and coordinating patient care for assigned caseloads.
Key Qualifications:
- Registered nurse with current licensure to practice professional nursing in the state.
- Graduate of a National League for Nursing accredited school of nursing.
- Maintain a current CPR certification.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
Preferred Qualifications:
- Minimum of two years' experience, at least one of which is in the area of public health or home care nursing.
Skills Required:
- Self-directed and able to work with minimal supervision.
- Demonstrates excellent observation, problem-solving, verbal, and written communication skills, nursing skills per competency checklist.
- Shows ability to organize and prioritize workload independently.
- Management experience not required. Responsible for supervising home health aides.
- Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.
