Medical Professional for Chronic Illness Care

We are seeking a skilled Nurse Practitioner to provide high-quality medical care to seniors and individuals with complex needs.
Key Responsibilities- Conduct comprehensive medical examinations to diagnose and treat chronic and urgent illnesses, minor injuries, and other health issues.
- Order and interpret laboratory and diagnostic testing as needed to ensure accurate diagnoses.
- Collaborate with healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
- Provide referrals to other healthcare practitioners when necessary to ensure patients receive the best possible care.
- Maintain up-to-date patient medical records in accordance with HIPAA and OSHA regulations.
- A valid state Nurse Practitioner license.
- Current National Board Certification as a Nurse Practitioner.
- A Master's degree in Nursing (MSN).
- No less than five years of nursing experience, with at least two years of experience as a Nurse Practitioner.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Excellent time management and organizational skills, with the ability to prioritize tasks and manage daily workflow.
- Demonstrated proficiency in electronic medical record (EMR) systems and Microsoft Office applications.
- A valid driver's license and automobile insurance.
