**The Role**
We are seeking an experienced Clinical Specialist - HIV Management to join our team. As a key member of our multidisciplinary team, you will be responsible for managing the health of patients with multiple chronic medical conditions, including HIV. You will work closely with patients, caregivers, and other healthcare professionals to develop and implement personalized care plans that promote optimal health outcomes.

Responsibilities:
  • Manage and improve health for patients with multiple chronic medical conditions, such as HIV, Diabetes, HTN, COPD/Asthma, CHF, and Chronic Renal Failure.
  • Work collaboratively with a team of healthcare professionals, including Nursing, Medical Assistant, Social Work, Behavioral Health, Pharmacy, Nutrition, etc., to provide comprehensive care.
  • Develop strong transitional care plans and track paneled patients along the continuum of care, including discharges from the hospital, ER, skilled nursing facilities/nursing homes, and home care.
  • Treat acute exacerbations of chronic conditions and provide education on healthy choices, disease progression, and advanced directives.
  • Track patients and their disease processes to prevent ER visits, admissions to the hospital, and readmissions.
  • Educate patients on their chronic diseases and encourage healthy lifestyle choices.

About You
We are seeking an experienced professional who has a passion for delivering high-quality patient care. The ideal candidate will have:
  • A Georgia Licensed Nurse Practitioner or Physician Assistant certification.
  • Experience working with patients who have multiple chronic medical conditions, including HIV, Diabetes, HTN, COPD/Asthma, CHF, and Chronic Renal Failure.
  • American Heart Association BLS certification.
  • Experience managing Medicare, Medicaid, and Commercial populations.
  • Proficiency in EMR systems, preferably eClinicalWorks, with notes completed within 48 hours.
  • Knowledge of chronic medical conditions and Medicare, Medicaid, and Commercial populations.
  • Excellent organization and communication skills.

Estimated salary range: $80,000 - $110,000 per year.
