Care Manager Extender

We are seeking a highly motivated and compassionate Care Manager Extender to join our team at RHA Health Services. As a Care Manager Extender, you will play a vital role in providing whole person, integrated care management services to individuals with intellectual and developmental disabilities, mental health, and/or substance use needs.
Key Responsibilities:- Perform general outreach, engagement, and follow-up with members to ensure they receive the necessary support and services.
- Coordinate services and appointments, including arranging transportation and providing wellness reminders.
- Engage in health promotion activities and knowledge sharing to support individuals in achieving their health goals.
- Share information with the care team and other stakeholders to ensure a comprehensive understanding of each individual's needs and circumstances.
- Provide and track referrals, and assist individuals in obtaining and maintaining community-based resources and social support services.
- Participate in case conferences and support the care manager in assessing and addressing unmet health-related resource needs.
- Valid driver's license and reliable transportation.
- High school diploma or equivalent, and at least 18 years of age.
- Must meet one of the following requirements:
- Be a person with lived experience with an I/DD or a TBI with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system.
- Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist.
- Be a parent or guardian of an individual with an I/DD or a TBI or a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual.
- Have two (2) years of paid experience performing the following types of functions:
- Performing general outreach, engagement, and follow-up with members.
- Coordinating services/appointments.
- Engaging in health promotion activities and knowledge sharing.
- Sharing information with the care team and other stakeholders.
- Providing and tracking referrals.
- Assisting individuals in obtaining and maintaining community-based resources and social support services.
- Participating in case conferences.
- Supporting the care manager in assessing and addressing unmet health-related resource needs.
- Competitive pay range of $15.00-$22.00 per hour.
