Care Manager Extender Job Description

We are seeking a highly motivated and compassionate Care Manager Extender to join our team at RHA Health Services. As a Care Manager Extender, you will play a vital role in providing whole person, integrated care management services to individuals with intellectual and developmental disabilities, mental health, and/or substance use needs.

Key Responsibilities:
  • Perform general outreach, engagement, and follow-up with members to ensure they receive the necessary support and services.
  • Coordinate services and appointments, including arranging transportation and providing wellness reminders.
  • Engage in health promotion activities and knowledge sharing to support individuals in achieving their health goals.
  • Share information with the care team and other stakeholders to ensure a comprehensive understanding of each individual's needs and circumstances.
  • Provide and track referrals, and assist individuals in obtaining and maintaining community-based resources and social support services.
  • Participate in case conferences and support the care manager in assessing and addressing unmet health-related resource needs.
Requirements:
  • Valid driver's license and reliable transportation.
  • High school diploma or equivalent, and at least 18 years of age.
  • Must meet one of the following requirements:
    • Be a person with lived experience with an I/DD or a TBI with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system.
    • Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist.
    • Be a parent or guardian of an individual with an I/DD or a TBI or a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual.
    • Have two (2) years of paid experience performing the following types of functions:
      • Performing general outreach, engagement, and follow-up with members.
      • Coordinating services/appointments.
      • Engaging in health promotion activities and knowledge sharing.
      • Sharing information with the care team and other stakeholders.
      • Providing and tracking referrals.
      • Assisting individuals in obtaining and maintaining community-based resources and social support services.
      • Participating in case conferences.
      • Supporting the care manager in assessing and addressing unmet health-related resource needs.
