About the Role

At Texas Health Action, we are seeking a highly motivated and compassionate Patient Advocate to join our team at our Kind Clinic location in San Antonio. As a Patient Advocate, you will play a vital role in optimizing the patient experience, helping them navigate the complex healthcare system, and providing support and guidance to ensure they receive the best possible care.

Key Responsibilities
  • Support patients in accessing HIV biomedical interventions for prevention and care services
  • Provide direct care to individuals, including conducting intake and assessments, providing sexual health education and counseling, and navigating insurance and benefits
  • Identify and address psychosocial barriers to accessing care and facilitate linkage to supportive services and relevant programs
  • Monitor issues affecting client access, adherence, and retention in care
  • Collect and analyze data to identify program progress and manage clinic operations using quality improvement tools
  • Act as a liaison between local agencies and the program, interpreting program guidelines and policies
  • Participate in program quality improvement and assist in making required adjustments and promoting program innovation
  • Maintain confidentiality and adhere to HIPAA compliance
Requirements
  • High school diploma or equivalent
  • 2+ years of full-time work/volunteer experience in serving diverse populations, including LGBTQIA communities and communities of color
  • Demonstrated experience taking initiative and exercising independent judgment, decision-making, and problem-solving
  • Proficient using computer software applications, such as Microsoft Word, Outlook, and Excel
  • Strong written and verbal communication abilities
  • Bilingual in English and Spanish
Preferred Qualifications
  • Bachelor's degree related to a human services field, such as social work, psychology, counseling, or public health
  • 1+ year(s) of experience working in HIV prevention and/or serving people living with HIV
  • Knowledge of insurance navigation
  • Familiarity and experience using a trauma-informed approach to working with individuals
  • Ability to work harmoniously with diverse groups of individuals
