Clinical Care Specialist

Job Summary: We are seeking a skilled Clinical Care Specialist to join our team at Samaritan Health Services. As a key member of our healthcare team, you will serve as an advocate, educator, and coordinator of services for at-risk patients.
About the Role: In this role, you will work closely with our board-certified plastic surgeons to provide high-quality care and personal attention to our patients. Your expertise will be essential in coordinating services, educating patients, and advocating for their needs.
Responsibilities:
- Coordinate patient care services, including scheduling appointments and managing patient records.
- Provide education and support to patients and their families, addressing their concerns and needs.
- Act as an advocate for patients, ensuring their rights and dignity are respected.
- Collaborate with healthcare professionals to develop and implement patient care plans.
- Participate in quality improvement initiatives to enhance patient care and services.
Requirements:
- High school diploma or equivalent required.
- One (1) of the following required:
- Bachelor's degree in a health-related field and one (1) year experience in a healthcare setting.
- Associate degree or successful completion of a technical program, plus a Registered or Certified Medical Assistant and two (2) years' experience in a healthcare setting.
- Three (3) years experience in a healthcare setting and two (2) years professional customer service experience.
- Healthcare Provider Level BLS required within 90 days of hire.
- Certification or Registration as a Medical Assistant preferred.
Preferred Qualifications:
- Experience in outpatient, ambulatory care, and/or community healthcare.
- Care coordination.
- Primary care related setting.
- Chronic conditions.
- Electronic medical information systems.
- Bilingual preferred.
