At Samaritan Health Services, we are seeking a skilled Healthcare Coordination Specialist to support our Population Health clinical and operational activities. This role will work closely with department clinical staff to ensure seamless care coordination for members and caregivers.

Job Summary

  • This position is responsible for assisting in the coordination of healthcare services for members, including identifying appropriate resources and eliminating barriers to care.
  • The ideal candidate will possess strong communication skills, both written and verbal, and be able to work effectively with all levels within the organization.

Key Responsibilities:

  • Care Coordination: Assist in coordinating healthcare services for members, including referrals to specialists, hospitalizations, and other medical treatments.
  • Case Management: Work with patients and families to develop individualized care plans that address their unique needs and goals.
  • Communication: Communicate effectively with healthcare providers, members, and families to ensure timely and high-quality care.
  • Data Analysis: Analyze data to identify trends and opportunities for improvement in population health outcomes.

Requirements:

  • Bachelor's degree in a health-related field or equivalent experience in healthcare or case management.
  • Medical Assistant certification preferred.
  • Experience with electronic documentation systems and Microsoft Office software.

Physical Demands:

  • Frequent standing and walking on a level surface.
  • Occasional lifting of up to 20 pounds.
  • Frequent rotating of the trunk while standing and sitting.
  • Frequent reaching forward and upward.
  • Occasional manual dexterity and finger dexterity.

Salary Range:

$55,000 - $65,000 per year, depending on experience.
