RN Hospice Care Specialist

Lumina Hospice and Palliative Care is seeking a skilled RN Hospice Care Specialist to provide compassionate care to patients and their families. As a Per Diem Hospice Nurse, you will play a vital role in delivering high-quality patient care and supporting the interdisciplinary team.
Responsibilities:
- Assess and evaluate patient needs, coordinating resources to meet those needs.
- Provide nursing care as needed, attentive to physical, emotional, and spiritual needs of patients.
- Counsel patients and families regarding symptom control, medications, and direct nursing care measures.
- Assist patients and families in making informed decisions about their care.
- Collaborate with other healthcare providers to ensure optimal patient care.
- Communicate effectively with patients, families, and healthcare team members.
- Develop and revise patient care plans in collaboration with the interdisciplinary team.
Qualifications:
- Current unencumbered Oregon licensure as a Registered Nurse.
- Professional nursing experience in hospice, palliative care, or a related field.
- Valid driver's license and reliable transportation.
- Excellent communication and documentation skills.
- Ability to work independently and as part of a team.
- Flexibility to work varied shifts, including days, evenings, and weekends.
