Role Summary:

As a Home Care Coordinator, you will play a crucial role in assessing the needs of elderly participants within their home settings. You will work closely with the Director of Clinical Services or Home Care Supervisor to develop and implement personalized care plans that align with the interdisciplinary team's objectives.

Key Responsibilities:
  • Conduct thorough assessments of participants in their home environments to identify individual care needs.
  • Collaborate with the interdisciplinary team to create and execute comprehensive care plans.
  • Act as a vital link between the PACE program and contracted home health services.
  • Maintain accurate and detailed medical records, documenting participant progress and changes.
  • Engage in quality improvement initiatives and contribute to team meetings.
  • Ensure that in-home staff deliver compassionate and effective personal care in accordance with established care plans.
  • Manage homecare staffing requirements to meet the needs of enrolled participants.
  • Adhere to all safety guidelines and company policies, maintaining confidentiality at all times.
Qualifications:
  • High School diploma or equivalent is required.
  • Preferred qualifications include a licensed nurse (RN or LPN) or certified Home Health Aide.
  • Basic Life Support (BLS) certification is required.
  • A valid driver's license and proof of auto insurance are necessary.
  • At least one year of experience working with the elderly population is essential.
  • Strong communication skills and a commitment to providing exceptional care are crucial.
