Role Summary:

As a Home Care Coordinator, you will work under the guidance of the Director of Clinical Services or Home Care Supervisor. Your primary responsibility will be to conduct assessments of participants in their home settings and coordinate individualized care plans with the necessary resources.

Key Responsibilities:
  • Conduct assessments to evaluate the needs of participants in their home environments.
  • Collaborate with the interdisciplinary team to develop and implement tailored care plans.
  • Serve as the primary liaison between the PACE program and contracted home health agencies.
  • Maintain accurate and comprehensive medical records for each participant.
  • Communicate changes in participants' conditions and progress to relevant staff members.
Qualifications:

Essential Skills:

  • Passion for serving elderly individuals in a community-based setting.
  • Strong communication skills with a focus on respect and positivity.
  • Knowledge of healthcare challenges faced by the elderly population.
  • Ability to identify safety hazards in home environments and propose improvements.
  • Commitment to delivering high-quality care and services.

Education and Experience:

  • High School diploma or equivalent is required.
  • Preferred qualifications include a nursing license (RN or LPN) or certification as a Home Health Aide.
  • Basic Life Support (BLS) certification is required.
  • A valid driver's license and proof of auto insurance are necessary.
  • Minimum of one year of experience working with elderly individuals is required.
General Duties:
  • Participate actively in interdisciplinary team meetings and quality improvement initiatives.
  • Ensure that in-home staff provide appropriate personal and restorative care based on established care plans.
  • Manage homecare staffing needs to align with participant requirements.
  • Adhere to all company policies and OSHA safety guidelines.
  • Maintain confidentiality regarding all company procedures and participant information.
