About the Role

We are seeking a skilled Patient Care Coordinator to join our team at Ascension. As a key member of our clinical integration team, you will play a vital role in ensuring seamless patient care throughout the continuum.

Key Responsibilities
  • Act as a liaison between management staff, administration, physicians, managed care companies, community organizations, and other stakeholders to facilitate effective communication and coordination of patient care.
  • Coordinate efforts to promote quality improvement and educational initiatives, ensuring that patients receive the highest level of care.
  • Compile and analyze data to track outcomes and make recommendations for process improvement, driving continuous quality enhancement.
  • Provide referrals and support for follow-up care for patients who are uninsured or underinsured, ensuring that they receive the necessary resources and services.
Requirements

To be successful in this role, you will need:

  • A Licensed Registered Nurse credential from the Indiana Board of Nursing, obtained prior to hire date or job transfer date.
  • A BLS Provider certification from the American Heart Association or American Red Cross, preferred.
