Job Summary

We are seeking a highly skilled and compassionate Home Coordinator to join our team at Caregiver, Inc. This is a full-time position responsible for providing assistance to the Residential Service Director, Area Director, and staff on site by ensuring quality assurance, compliance with program principles, and a safe and healthy environment for individuals served.

Key Responsibilities
  • Quality Assurance and Compliance: Ensure the delivery of residential services by Residential Support Staff Team Members, ensuring the health and safety of individuals served and staff, and ensuring Residential Support Staff Team Members are trained in individual treatment and training plans.
  • Operations Management: Coordinate transportation services for individuals when needed, oversee maintenance of assigned homes and vehicles, and provide assistance with medications and perform tasks delegated by a Registered Nurse.
  • Program Development and Implementation: Ensure programmatic needs of individuals are met by observation of direct care staff and daily review of programmatic documentation, responsible for ensuring direct care staff follow PDPs as written and provide coaching and counseling when needed.
  • Supervision and Training: Supervise Residential Support Staff Team Members, ensure that home is fully staffed, and complete monthly monitoring of Residential Support Team Members' competency of required programming and medical needs for each of the individuals residing in the home.
Requirements
  • Education and Experience: Minimum of 2 years working with individuals with developmental disabilities, valid Indiana Driver's license, good driving record, and valid insurance on any personal vehicle, High School Diploma or GED required, Associate degree in any field preferred.
  • Skills and Abilities: Good communication skills, knowledge of the HCS and/or ICF-IID programs is preferred, ability to handle multiple tasks efficiently and effectively, ability to perform all functions as stated in the Home Coordinator job description, and ability to operate various computer systems.
