Job Summary

Patient Centered Services LLC is seeking a compassionate and skilled Pediatric Home Health Nurse to provide high-quality care to pediatric patients in Washington, DC. The ideal candidate will hold a valid DC nursing license, current AHA BLS certification, and have experience in Ventilator, Tracheostomy care, and/or Gastrostomy tube (G-tube) management.

Key Responsibilities
  • Deliver exceptional nursing care to pediatric patients in a home care or clinical setting.
  • Perform assessments, administer medications, and execute treatment plans as directed by healthcare providers.
  • Monitor patient conditions, document updates, and communicate changes to the supervising RN, physician, or care team.
  • Provide education and support to patients and families regarding disease prevention, management, and care techniques.
  • Collaborate with interdisciplinary teams to ensure a patient-centered care approach.
Requirements
  • Active DC Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
  • Current American Heart Association (AHA) BLS certification (CPR & AED).
  • Experience in Ventilator, Tracheostomy care, and/or Gastrostomy tube (G-tube) care required.
  • Strong interpersonal and communication skills.
  • Ability to work independently and in collaboration with other healthcare professionals.
Preferred Qualifications
  • Pediatric nursing experience highly preferred.
  • Advanced certifications related to pediatric care are a plus.
