Job Summary

The Crossover Health Nurse plays a critical role in creating a positive patient experience. This position is responsible for coordinating and connecting patients with specialists and their facilities, ensuring that patients receive the highest level of care. The ideal candidate will be a team player who is passionate about delivering exceptional patient care and is committed to our organization's values.

Key Responsibilities
  • Deliver Patient Care: Provide high-quality patient care, practicing under the oversight of an RN or clinic supervisor.
  • Facilitate Patient-Centered Care: Collaborate with patients and the care team to ensure that patients receive comprehensive and coordinated care.
  • Provide Evidence-Based Care: Deliver nursing care that is grounded in evidence-based medicine guidelines and meets the highest community standards.
  • Coordinate Specialist Care: Connect patients with specialists and their facilities, ensuring that patients receive timely and effective care.
  • Provide Phlebotomy and Immunization Services: Perform phlebotomy and immunization services as needed.
Requirements
  • Licensed Vocational Nurse: Must be a licensed vocational nurse from an accredited school with a minimum of 3 years of clinical experience.
  • BLS Certification: Must have current BLS (Basic Life Support) certification.

Physical Job Requirements
  • Standing, Walking, and Sitting: Requires standing, walking, and sitting for extended periods of time.
  • Lifting and Carrying: Occasionally lift and carry items weighing up to 50 lbs.
  • Manual Dexterity: Manual and finger dexterity and hand-eye coordination.
  • Full Range of Motion: Includes full range of body motion, including the potential of handling and lifting patients.
  • Corrected Vision, Hearing, and Speech: Requires corrected vision, hearing, and speech within normal ranges, with the ability to effectively communicate with patients and team members.
