Job Summary

We are seeking a highly skilled Nurse Practitioner to join our team of Advanced Practice Clinicians. As a Geriatric Care Specialist, you will be responsible for providing primary care to patients in their homes, working closely with our interdisciplinary team to deliver comprehensive care and support.

Key Responsibilities
  • Provide primary care to patients in their homes, including routine check-ups, urgent care, and acute medical care
  • Collaborate with our interdisciplinary team to deliver comprehensive care and support to patients
  • Conduct initial patient screenings and physical examinations, creating appropriate assessment and care plans
  • Order, interpret, and analyze diagnostic tests, determining the need for follow-up appointments and further treatment options
  • Prescribe and manage medications and treatments as appropriate
  • Update patient records and ensure accuracy at each patient appointment
  • Deliver quality care while maintaining all company metrics pertaining to documentation, coding, HEDIS measures, patient engagement, and medical expense
  • Serve as a resource for patients, delivering ongoing care information, counseling, and guidance for critical conditions
  • Collaborate with the interdisciplinary team and participate in regularly scheduled team meetings
  • Maintain required licensure and certification
  • Deliver palliative care and end-of-life care to patients and families, as needed

Requirements

To be successful in this role, you will need:

  • A Master's degree in Nursing and a current APRN license
  • Experience working in geriatric care and home-based medical care
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong clinical skills and knowledge of geriatric care
