About the Role

Fairview Health Services is seeking a skilled Licensed Mental Health Professional to join our Triage and Transition Department as a Telemedicine Crisis Counselor. In this role, you will provide high-quality crisis assessments and interventions to patients in need, working closely with our interdisciplinary team to ensure seamless care coordination.

Key Responsibilities
  • Conduct comprehensive behavioral assessments to identify risk and develop individualized treatment plans
  • Provide crisis call support and determine appropriate levels of care for clients
  • Collaborate with patients, families, and healthcare providers to develop and implement treatment plans
  • Document patient information and treatment plans in accordance with medical recordkeeping standards
  • Communicate effectively with internal and external stakeholders, including hospital emergency departments, schools, and primary care clinics
Requirements
  • Master's degree in Social Work, Psychology, or a closely related field
  • Two or more years of experience in crisis counseling or therapy situations
  • Experience serving patients with mental health, substance use, and co-occurring disorders
  • One of the following licenses in good standing in the State of Minnesota: Licensed Psychologist, Licensed Independent Clinical Social Worker, Licensed Marriage Family Therapist, or Licensed Professional Clinical Counselor
