Pharmacist - Anticoagulation Specialist

Confluence Health is seeking a highly skilled and experienced Pharmacist to join our team as an Anticoagulation Specialist. As a key member of our pharmacy team, you will be responsible for providing clinical and operational pharmacy services to patients in our ambulatory care setting.
Key Responsibilities- Manage medication therapy and order appropriate labs based on published medical literature, Confluence Health guidelines, evidence-based protocols, and individual patient clinical parameters.
- Enter progress notes with additional information about medication therapy management decisions.
- Ensure all orders placed by the pharmacist are pursuant to an established protocol initiated by the provider or co-signed by an authorized provider to ensure conditions of payment are met as necessary.
- Maintain competence in CPT coding and pass regular compliance audits.
- Communicate with patients, family members, and other healthcare providers to promote coordinated healthcare across the continuum.
- Advocate for and support formulary management and pharmacy and therapeutics initiatives.
- Participate in department and organizational performance initiatives.
- Provide direct supervision and oversight of clinical staff members.
- Commit to instructing, modeling, and coaching staff, students, and residents.
- Report adverse drug events and mitigate future patient safety risks through participation in medication quality activities.
- Demonstrate competency and proficiency in essential job functions by maintaining active credentialed status via Confluence Health delegated credentialing through the Medical Staffing Office.
- Required to maintain active Basic Life Support (CPR) certification.
- Bachelor of Science in Pharmacy (RPh).
- Licensed to practice in the state of Washington prior to start date.
- Desired qualifications include Pharm. D, bilingual ability to pass a bilingual fluency exam, and three years of experience as a clinical pharmacist in the ambulatory setting or successful completion of a pharmacy practice residency with ambulatory care experience.
The Pharmacist will work in a fast-paced ambulatory care setting and will be exposed to infectious patients or materials, hazardous and toxic substances, and potential musculoskeletal injuries if proper lifting and carrying techniques are not used.
