We are seeking a highly skilled Clinical Pharmacist to join our team at Confluence Health. As a Clinical Pharmacist, you will play a critical role in providing patient-centered care and optimizing medication therapy for our patients.
Key Responsibilities- Monitor and manage medication therapy for patients with complex medical conditions, including anticoagulation therapy.
- Collaborate with healthcare providers to develop and implement individualized treatment plans.
- Provide education and support to patients and their families on medication use and disease management.
- Conduct regular assessments of patient risk factors and adjust medication therapy as needed.
- Document patient care activities and communicate with healthcare providers through the electronic medical record.
- Doctor of Pharmacy degree from an accredited school of pharmacy.
- Licensed to practice pharmacy in the state of Washington.
- Strong computer skills and ability to work in multiple systems.
- Experience in clinical pharmacy practice, preferably in an ambulatory setting.
This position requires the ability to work in a fast-paced environment and maintain confidentiality and discretion when handling sensitive patient information.
