Job Overview

As a Home Care Licensed Practical Nurse (LPN) with Interim HealthCare of the Twin Cities, you will have the opportunity to work independently and autonomously in a home environment, providing one-on-one care to patients. This role offers a range of benefits, including a competitive pay rate, a $6,000 hiring bonus, mileage reimbursement, and a company car program.

Key Responsibilities:

  • Participate in ongoing interdisciplinary assessment of the patient.
  • Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s).
  • Provide services ordered by the physician as indicated in the plan of care.
  • Recognize and report changes in the patient's condition to the RN.
  • Counsel and educate the patient and family in meeting healthcare related needs.
  • Complete timely and accurate clinical notes.

Requirements:

  • Current license as an LPN in the State of MN.
  • 2 years' experience as an LPN.
  • Able to travel within the geographic area serviced by the office.
  • CPR Certification.
