Job Title: Licensed Practical Nurse (LPN)

About the Role:

We are seeking a skilled and compassionate LPN to join our team in providing patient-centered care in the home setting. As an LPN, you will be responsible for:

  • Participating in ongoing interdisciplinary assessment of the patient
  • Developing and evaluating the plan of care in partnership with the patient, representative, and caregiver(s)
  • Providing services that are ordered by the physician as indicated in the plan of care
  • Recognizing and reporting changes in the patient's condition to the RN
  • Counseling and educating the patient and family in meeting healthcare-related needs
  • Completing timely and accurate clinical notes, including addressing patient progress

Requirements:

To be considered for this role, you must:

  • Hold a current license as an LPN in the State of MN, with an active and good standing license
  • Have at least 2 years of experience as an LPN
  • Be able to travel within the geographic area serviced by our office, with a valid driver's license, reliable transportation, and proof of auto insurance
  • Hold CPR certification
