Job Summary:We are seeking a skilled and compassionate LPN to join our team of healthcare professionals. As an LPN, you will be responsible for providing high-quality patient care in the comfort of their own homes. Your duties will include:
- Participating in ongoing interdisciplinary assessment of the patient
- Developing and evaluating the plan of care in partnership with the patient, representative, and caregiver(s)
- Providing services that are ordered by the physician as indicated in the plan of care
- Recognizing and reporting changes in the patient's condition to the RN
- Counseling and educating the patient and family in meeting healthcare related needs
- Completing timely and accurate clinical notes including addressing patient's progress
To be considered for this role, you must have:
- A current license as an LPN in the State of MN
- 2 years' experience as an LPN
- Able to travel within the geographic area serviced by the office
- Valid driver's license, reliable transportation, and proof of auto insurance
- CPR Certification
We offer a competitive compensation package, including:
- Pay Range: $35.88/hour
- Hiring bonus: $6,000
- Mileage Reimbursement / Company Car Program

This position will work in the field helping patients in their home setting. Our office is located in Roseville, MN, and the general territory for this position would be the Southwest Twin Cities Metro area.
