Community Health Worker

Sea Mar Community Health Centers is seeking a highly motivated and compassionate Pathways Community Health Worker to join our Care Management team in Tumwater, WA.
Job Summary:
As a Pathways Community Health Worker, you will work closely with clients, community organizations, and other departments to provide culturally sensitive and geographically connected support. You will assist clients in overcoming barriers to health, social services, education, and employment through home visiting and other outreach efforts.
Key Responsibilities:
- Conduct standardized patient intake and risk assessments to identify client needs and develop strategies to address or eliminate barriers to care.
- Establish trusting relationships with clients and community partners to facilitate access to resources and services.
- Work collaboratively with the Program Manager, PCP, and other assigned personnel to ensure that all relevant client needs are identified and addressed.
- Participate in ongoing assessment of client and family needs to determine if expected outcomes are reached and to ensure implementation and/or adjustment of the service plan.
Requirements and/or Experience:
- High School Diploma or Equivalent required.
- Community outreach or case management experience preferred.
- Experience working with people from various backgrounds required.
- Experience working with individuals with substance use disorders, homelessness, chronic mental illness, or people in crisis preferred.
- Prefer one year paid or volunteer experience providing medical, social service, or behavioral health case management or care coordination.
- Bilingual English/Spanish is preferred, but not required.
