Key Responsibilities
  • Schedule Appointments and Coordinate Care

Work closely with patients, healthcare providers, and other stakeholders to schedule appointments and coordinate care. Ensure that patients receive timely and effective care, and that their needs are met.

Provide Patient Support and Education

Offer patients support and education to help them manage their health conditions, understand their treatment options, and make informed decisions about their care. Provide patients with information and resources to help them navigate the healthcare system.

Collaborate with Healthcare Teams

Work collaboratively with healthcare teams, including physicians, nurses, and other healthcare professionals, to ensure that patients receive comprehensive and coordinated care.

Identify and Address Health Disparities

Identify and address health disparities and barriers to care, and work to eliminate them. Develop and implement strategies to improve health outcomes and reduce health inequities.

Participate in Quality Improvement Initiatives

Participate in quality improvement initiatives to ensure that patients receive high-quality, patient-centered care. Identify areas for improvement and develop strategies to address them.

Requirements
  • Education and Experience

A high school diploma or equivalent is required. A degree in a healthcare-related field, such as nursing or health education, is preferred. At least one year of experience in a healthcare setting is required.

Skills and Abilities

Excellent communication and interpersonal skills are required. Ability to work effectively in a fast-paced environment and to prioritize tasks and manage time efficiently. Ability to maintain confidentiality and handle sensitive information with discretion.

Language Skills

Bilingual English/Spanish skills are preferred, but not required.
