Transition Specialist in Adult Services

**Job Overview:**
Hopewell is seeking a highly skilled **Transition Specialist in Adult Services** to join our team.
In this role, you will develop positive relationships with individuals and caregivers, ensuring that individuals we support are treated with dignity and respect in accordance with our Human Rights policy.
The ideal candidate will possess:
- BS/BA in a human services field preferred, with 2 years' experience working with individuals with intellectual disabilities, preferably in a Shared living setting.
- Ability to communicate effectively verbally and in writing, and use good judgment.
- Demonstrated ability to be flexible, resourceful, and juggle multiple tasks and priorities.
- Computer skills to perform essential functions, strong organizational skills, and time management skills.
**Responsibilities:**
- Assure high-quality support and comprehensive training is provided to individuals through a Positive Behavior Support (PBS) approach.
- Collaborate with our Director to complete referrals and intakes, ensuring appropriate placements are made.
- Provide on-call coverage and crisis management, ensuring individuals' well-being and notifying relevant stakeholders.
