As a **Care Coordinator for Intellectual Disabilities**, you will play a vital role in our mission by developing partnerships and providing guidance to meet the evolving needs of our individuals as they age.

This is an exciting opportunity to work collaboratively with our team to support, develop, and grow our Shared Living program.

**Key Responsibilities:**

  • Manage high-quality placements, supporting individuals in our Shared Living and In-Home Support programs to live a happy, healthy, and productive life.
  • Implement industry best practices, create innovative solutions to complex problems, and employ staff who care deeply about individuals, our mission, and values.
  • Collaborate with our Director to engage in recruitment, selection, matching, transition, retention, and evaluation of caregivers.
  • Provide on-call coverage and crisis management, ensuring individuals' well-being and notifying relevant stakeholders.

**Requirements:**

  • BS/BA in a human services field preferred, with 2 years' experience working with individuals with intellectual disabilities, preferably in a Shared living setting.
  • Ability to communicate effectively verbally and in writing, and use good judgment.
  • Demonstrated ability to be flexible, resourceful, and juggle multiple tasks and priorities.
  • Computer skills to perform essential functions, strong organizational skills, and time management skills.
  • Empathy and desire to work with adults with developmental disabilities.

**Salary:** $55,000 - $65,000 per annum, depending on experience.
