Job Overview

As a Transition Coordinator at HopeWell Inc. in Eastern, MA, you will play a vital role in supporting adults with intellectual disabilities to live happy, healthy, and productive lives. We are seeking an experienced professional to manage high-quality placements in our Shared Living and In-Home Support programs.

About the Role

This is an exciting opportunity to work collaboratively with our Director, Home & Community-Based Adult Services, to support, develop, and grow our Shared Living program. You will be responsible for ensuring that individuals receive comprehensive training and support through a Positive Behavior Support (PBS) approach, increasing their independence. Additionally, you will collaborate with the Director to engage in recruitment, selection, matching, transition, retention, and evaluation of caregivers, as well as provide on-call coverage and crisis management.

Requirements

We are looking for a candidate with a BS/BA in a human services field, preferably with 2 years' experience working with individuals with intellectual disabilities, preferably in a Shared living setting. You should have excellent communication skills, both verbally and in writing, and be able to use good judgment. The ideal candidate will also possess strong organizational skills, time management skills, empathy, and a desire to work with adults with developmental disabilities. A valid MA State Driver's License and daily access to an automobile are required. $63,000 - $73,000 per year.
