Job Title: Physical Nutritional Management Plan Coordinator

Job Summary:

We are seeking a highly skilled and dedicated Physical Nutritional Management Plan Coordinator to join our team at the Texas Department of Aging & Disability Services. As a key member of our staff, you will be responsible for providing exceptional care and support to individuals with intellectual and developmental disabilities.

Key Responsibilities:

* Conduct competency-based training of staff on physical and nutritional management plans
* Monitor programs and ensure compliance with established protocols
* Provide input into the development of individualized plans to meet the unique needs of each person
* Collaborate with the Nutritional Management Team to ensure accurate and effective implementation of plans

Requirements:

* LPN licensure and experience in direct care
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
