About the Role

We are seeking a skilled Physical Therapist Case Manager to join our team at 21st Century Home Health Services Inc. As a Physical Therapist Case Manager, you will play a vital role in delivering high-quality home health care to our patients.

Key Responsibilities
  • Conduct thorough evaluations in the patient's home to develop personalized care plans.
  • Work collaboratively with Physical Therapy Assistants to implement care plans and achieve patient goals.
  • Communicate effectively with patients' physicians to ensure seamless care coordination.
  • Evaluate and report patients' progress to the medical director.

Requirements
  • Current unrestricted California Physical Therapist License.
  • Current BLS/CPR Certification.
  • Valid California Driver's License.
  • Knowledge of local community resources and state and federal homecare regulatory guidelines.
