Physical Therapist Home Health - Weekender

VNA Health Group is seeking a dedicated and compassionate Physical Therapist to join our team in Warrensville Heights, Ohio.

Job Summary:

We are looking for a Physical Therapist to provide high-quality patient care in a home health setting. The ideal candidate will have a strong passion for delivering exceptional care and a commitment to excellence.

Responsibilities:

  • Conduct thorough assessments and evaluations of patient care needs
  • Develop and implement individualized treatment plans to improve patient outcomes
  • Provide direct physical therapy services, including instruction and education to patients and caregivers
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care
  • Document patient progress and outcomes in a timely and accurate manner

Requirements:

  • Bachelor of Science Degree in Physical Therapy required, post-graduate degree preferred
  • One year experience in a supervised healthcare setting
  • Licensed, in good standing, to practice physical therapy in the State of Ohio
  • CPR certification required
  • Valid Driver's License
