Job Title: Physical Therapist Home Health

VNA Health Group is seeking a skilled Physical Therapist to join our Home Health team in Warrensville Heights, Ohio.

About the Role:

We are looking for a compassionate and dependable Physical Therapist to work with our patients in their homes, providing ongoing care and support. As a Physical Therapist Home Health, you will be responsible for assessing and evaluating patient care needs, developing treatment plans, and performing interventions to improve patient well-being.

Responsibilities:
  • Perform initial assessments and determine patient treatment needs
  • Develop plans of care with measurable goals and specific discipline-related treatments
  • Provide direct physical therapy services, including performance of physical therapy procedures and instruction of patients and caregivers
  • Re-evaluate and revise plans of care as needed with physician approval
  • Facilitate total care services to patients by making appropriate referrals to other disciplines
Qualifications:
  • Bachelor of Science Degree in Physical Therapy required, post-graduate degree preferred
  • One year experience in a supervised healthcare setting
  • Licensed, in good standing, to practice physical therapy in the State of Ohio
  • CPR certification required
